The Elliot VNA works with Elliot Palliative Care Service in proving for Palliative Care Medical consultations.

Referral Capacity & Service Availability
Our commitment to safe, high‑quality care
We are committed to accepting every appropriate referral we can safely serve. To do that, we continuously monitor our census, staffing and clinician caseloads and complexity to determine referrals which we have capacity to accept. This can change day to day. Our goal is to match each patient with the right level of care, at the right time, in the safest setting.
Service availability and visit frequency
Because home health and hospice are delivered in the community, our ability to accept new referrals depends on current staffing and visit demand across our service area.
- Visit frequency expectations: We are designed for intermittent, skilled visits. We are unable to support daily or more frequent services without a predictable end date and/or patient/caregiver willingness to learn. In these situations, we work with referring providers to explore alternative levels of care or additional in‑home support.
- Licensed Nursing Assistant (LNA) services: At this time, we are unable to support twice‑daily Licensed Nursing Assistant services. When more intensive personal care is needed, we collaborate with families and community partners to identify other options.
You may occasionally experience longer waits for specific disciplines (for example, nursing, physical therapy, occupational therapy, speech therapy, social work, or LNA services) depending on staffing and community demand.
Clinical appropriateness and service complexity
Some needs are better met in settings with continuous monitoring, specialized equipment, or intensive behavioral health support. In those cases, home health or hospice may not be the safest or most effective option.

If you have questions about a specific referral, service intensity, or clinical scenario, our intake team is available to discuss options in real time. We welcome the opportunity to collaborate so that patients receive the right care, in the right place, at the right time, even when that means connecting them to another level of care.
