The Elliot VNA Palliative Care program offers healthcare services designed for those with life limiting conditions that wish to continue to seek treatment and/or cure, but who also want to maintain their quality of life and manage their symptoms. The Elliot VNA works with Elliot Palliative Care Services in providing Palliative Care Medical consultations.

Collaborative Care Program
This program supports patients living with serious or chronic illness, who need help managing pain and symptoms. Our team of experts work directly with a comprehensive list of providers, to improve comfort and ensure seamless coordination and guidance through every stage of care.
